ya its an odd thing coming from a band that uses band member illness as an excuse to cancel strings of shows.

Probably, because if worse comes to the worse.
Most of the songs, could probably be played with 2 guitarists or Izzy plays a show and DJ moves to Ron's parts.
Not making some cyncial or reunionst comment, it's what I'd assume could happen.

Has he been complaining about the pain this tour
From what I understand he's been in a state of constant pain since his car accident.
I think replacing him will be the last thing on anyone's mind for three reasons:
1. The band seem pretty close. I'd imagine their instinct would be to stand by him and help him get through rather than boot him while he's down.
2. Ron's a very good package for the band. He's well liked by fans, has stood the test of time by hanging around for 5 years or whatever now, he does the media well, and he's got insane shred skills. He's often been ranked amongst the 50 fastest guitarists in the world, and some list him top 10. There's other people who can cover buckethead but it's not simply a case of grabbing anyone off the street to do it...especially when if you think about it the person needs to have that level of talent and also not be in a band or running a solo career that can't be made secondary to GNR.
3. Even if they were to replace him. There's a retooling involved. The whole stage show has each song split into 3 parts - so someone would have to learn which parts of what solos are his bits and all the associated stage blocking and so on. Doable...but not something you'd want to do mid tour. It's more like a break would occur and a new line up would surface next tour.
Anyway...with any luck Ron will find some form of treatment or solace somewhere that can keep him sane, provide an acceptable lifestyle and keep him in the band. Pain of that level is one of the hardest things.
